✔ A Statement of Good Faith: A declaration, under penalty of perjury, that you have a good faith belief that the content was removed due to a mistake or misidentification.
✔ Consent to Jurisdiction: A statement agreeing to the jurisdiction of your local federal court or a legal representative.
✔ Your Physical or Electronic Signature.
